Chronic Disease Management Scheme (CDM)

If your GP deems your condition to be chronic or you have other ongoing health concerns, they may issue you with an CDM form at their discretion.

This enables you to have up to 5 physiotherapy sessions with us at Avalon Physiotherapy & Clinical Pilates, which will be mostly covered by Medicare. These sessions must only be used within twelve months of the date of issue.

In addition, we can claim your rebate from Medicare on the spot. Just be sure to bring your Medicare card along with a debit (EFTPOS) card and we will take care of the rest.

Unfortunately, the process does not work with a credit card.
