If your GP deems your condition to be chronic or you have other ongoing health concerns, they may issue you with an CDM form at their discretion.<\/p>\n
This enables you to have up to 5 physiotherapy sessions with us at Avalon Physiotherapy & Clinical Pilates, which will be mostly covered by Medicare. These sessions must only be used within twelve months of the date of issue.<\/p>\n
In addition, we can claim your rebate from Medicare on the spot. Just be sure to bring your Medicare card along with a debit (EFTPOS) card and we will take care of the rest.<\/p>\n
Unfortunately, the process does not work with a credit card.<\/p>\n
